No longer will you have to worry about losing your immunization records or having to receive unneccessary duplicate immunization shots.  You will be able to request this infomation as often and at anytime you desire. 

Signing up is easy as 1,2,3.   Follow these steps:

   Fill out Basic Info Form...  click here

  Submit Payment....  click here

  Mail us a Copy of Shot Records....  click  here

Once all three items are submited (Basic Info Form, Payment, and Shot Record Detail), we will have a Shot History.com associate contact you with your personal account ID and account confirmation.
